Room Change Request Form (86k PDF*) » Any student wishing to change his or her room assignment must complete this form before changing rooms. The Room Change Request Form will be available after the 2-week room freeze.

Pet Registration Form (102k PDF*) » This document includes the current pet policy, which must be adhered to for any student wishing to bring a pet to campus.

Loft Policy and Waiver of Liability (42k PDF*) » Any student who constructs his/her own loft must complete this form and agree to the construction guidelines therein. The loft policy may also be found in the OWU Student Handbook, item #14.

Meal Plan Change Form » During the first two weeks of the semester, any student may request to change the meal plan that he/she signed up for. Changes may not be made after 2 weeks into the semester.
